Initial Evaluation​ $120/hour

An Occupational Therapist will perform a detailed and comprehensive assessment including medical history, patient background, and current functional limitations to create an occupational profile related to the client's abilities, values, and life roles. The assessment includes direct discussion between the clinician and the client as well as standardized assessments that relate to the client's abilities and deficits. The purpose of the initial evaluation is to determine the most appropriate treatment interventions to assist the client in reaching his/her goals.

Client goals may include but are not limited to:

• improving independence with activities of daily living

• improving strength for safe performance of functional tasks

• addressing safety and fall prevention strategies

• home modifications to promote aging in place

 

Treatment Session​ $70/hour

Treatment sessions focus on client-centered interventions to improve functional independence and safety for daily tasks to promote aging in place. Includes a plan of care developed by the OT, standardized assessments, and documentation of progress.
